This year’s event will put a spotlight on the extraordinary work of our Adult Care Management Program—a vital service that supports men, women and families living with mental illness, chronic health conditions, and/or substance use challenges. Our care managers are quiet heroes who help people navigate complex systems, access essential resources, and rebuild their lives during these challenging times.
