Mobile Crisis Intervention Team

Full Time/Part Time M-F 9am-5pm

$78,000 annual for full time

$38.46/hr for part time

MHA Mobile Crisis Team is looking for a full-time or part-time LMSW/LMHC. The clinician will be working with multidisciplinary team and community-based agencies to provide clinical assessment and evaluation to youth, adults and families in immediate behavioral health crisis.

Daily responsibilities include:

· Conduct risk assessments, mental status examinations.

· Provide short-term crisis intervention and stabilization services to adults, children, adolescents and their families in the home or community (wherever the individual is experiencing a crisis).

· Analyze information to identify action plan, which includes crisis stabilization plan, safety and crisis prevention plans, and plan for follow-up.

· Consult with team and supervisor as appropriate. Document information in accordance with applicable standards.

· Coordinate with community resources for collateral input and follow-up care, including referrals for substance abuse assessment and treatment.

· Monitor implementation of action plan, including follow-up meetings with clients and collaterals.

· Conduct wellness checks (home/community visits) to identified clients.

· Plan for safety, crisis prevention/management, and continuing care

· Develops and communicates a plan of action, person-centered safety plan and recommendations in conjunction with their crisis responder team member and others involved.

· Assists with recommendations and referrals for the crisis responder case manager to conduct follow-up services to ensure continuity of care.

· Competently makes decisions about the need for an emergency pick up/transport order when an individual is at risk of harm to themselves or others. Works in conjunction with law enforcement and emergency personnel for transportation to a higher level of care.

· Collaborates with First responders, Health Homes, Hospitals and Community/treatment providers to develop an integrated plan of care including mental health, physical health, and substance abuse services.

Collaboration with community supports are necessary for individuals to succeed in the community and ensures true service integration and collaboration to allow for successful stabilization of individuals.

Master's degree in related clinical degree

Candidates must be Licensed or working toward licensure in any of the following: Licensed Psychologist; Licensed Psychoanalyst; Licensed Master Social worker (LMSW); Licensed Clinical Social Worker (LCSW); Licensed Marriage & Family Therapist(LMFT); Licensed Mental Health Counselor(LMHC).Must possess valid NYS driver's license and able to ride in a vehicle for extended periods of time. There will be extended periods of time walking.
