Children's Health Home Care Manager
Full Time 35 Hours a week
$25.00/hr to $27.00/hr
DESCRIPTION:
Applicants must have the capacity to work closely within a team setting to assist clients and families in Dutchess County with service coordination, referral to needed resources/interventions, and managing daily living activities. The Care Manager will assess service needs, liaison with community agencies and schools, coordinate treatment, and provide advocacy. Strong written and verbal communication as well as time management skills a must.
QUALIFICATIONS:
· Bachelor of Arts or Science in human services related field with two years of relevant experience, OR
· A License as a Registered Nurse with two years of relevant experience, OR
· A Masters in human services related field with one year of relevant experience
· Valid Driver’s License.
· Bilingual Spanish speaking a plus
