Individualized Support Programs

Are you attending (or about to attend) Dutchess Community College? Are you struggling with a psychiatric disability or an addiction disorder? Could you use a little extra support on campus?

Our Supported Education Program will allow you to make the most of your college experience by helping you get adjusted and connect to useful services. Achieve your educational goals, build strong relationships, find meaningful work, and stay happy and healthy throughout!

Are you (or someone you know) between 18 and 25 and struggling to adjust to adulthood? Is your teen currently in the Teen Challenge[LINK] program and looking for the next step?

The Young Adult Transition Program helps you understand what you need to grow up and make a plan to get there. You get a blend of life skills training, socialization, planning for the future, and fun!

Do you know a child that’s currently in foster care or at risk of placement? Do you wish you had someone neutral involved to provide an outside perspective with the kid’s best interests at heart? An objective point of view?

When your case is assigned a CASA worker, you get a trained volunteer advocate who works to observe and understand the child’s situation. This person provides Family Court judges with impartial information about the circumstances of the children involved.

Are you caring for someone under 21 with chronic and serious mental health conditions, including functional impairment? Are you struggling to juggle all the different providers, physicians, and support providers?

If you qualify for Medicaid, we can help you get a single Care Manager. This single Care Manager can help you make appointments and coordinate health and community health agencies to keep your child healthy and safe as simply as possible.

Are you struggling with mental illness but still wishing you could get out more? Want to spend time with people who understand what you’re going through?

Compeer matches adults with community volunteers to go for weekly outings and activities together. You get a supportive friend and mentor, who’s there to check in with you as much or as little as you need to relax and have fun.

Are you the parent or caregiver for a child with a Serious Emotional Disorder (SED) between the ages of 4 and 18? Do you ever feel burnt out or wish you could have a bit of a break to focus on your own life?

Our Respite program allows you as parent or caregiver to take some time for yourself. You get the opportunity to focus on self-care (or errands, or whatever!) while your child goes out with a trained respite worker for fun and educational activities.

Are you parent or caregiver to a child with emotional, social, or mental health challenges? Are you going through a particularly rough challenge and need some extra support?

Our Family Peer Advocate is here to listen to your story and work with you one-on-one to find a solution. Whether it’s finding you the appropriate resources or going to court, social services, or a hospital on your behalf, we’re here to help you get through this!

Do you have a mental health diagnosis, two or more chronic medical conditions, and/or substance abuse challenges? Do you have consistent daily challenges, but don’t know where to turn?

This program gives you a single point of access to manage all of your care providers, help assess areas where you need help, and provide you the care and resources to enrich your life.

Are you over 18 and suffering from or caring for someone with a psychiatric illness? Do you need help, but you’re not 100% sure what you need—for example in the case of moving or a sudden life upheaval?

Our Adult Advocacy program will get you the guidance, resource information, and support you need. Give us a call and we can link you with advocacy, referrals, or just a plan to help you move forward with dignity!

Are you or a family member a veteran in Dutchess County suffering from PTSD, suicidal thoughts, TBI, depression, addiction or other mental health issues? Do you need help and aren’t sure where to turn?

We are committed to helping you set personal recovery goals and then get there, whether it’s by making a plan, developing the skills you need, or just giving you support along the way.

Are you or a family member a veteran in Dutchess County suffering from PTSD, suicidal thoughts, TBI, depression, addiction or any other mental health issues? Do you feel lonely and wish you had one-on-one support to get your life on track?  

Compeer CORPS is a program which matches veterans with other veterans who are trained mentors, able to help you make and meet goals, engage in social activities, and access support services.

Are you living with a Severe Persistent Mental Illness (SPMI) and looking to become more independent and comfortable in society?

This program will help you learn and refine your life skills. You get the opportunity to build your own schedule, learn in a familiar setting, and take the pressure off by learning coping skills and medication management.
