Case Manager
Full Time 35 hours week. Location: Stabilization Center, 230 North Road
Monday - Friday 9am-5pm

DUTIES:
Obtain mental health appointments for guests and fax referrals as needed.
Support CASACs and Clinicians in checking for bed availability.
Helping guests complete intake paperwork to various detox facilities and rehab facilities.
Transporting guest to next level of care.
Support guests while on screening for intake appointments.
Setting up Medicaid transportation.
Communicating with the guest’s case manager.
Must be able to work with an interdisciplinary team.
QUALIFICATIONS:
Four years experience working in a direct care setting in the human service field preferred.
Associates Degree preferred.
Knowledge of the services available in Dutchess County.
Valid NYS Driver’s license
